Psychiatric Home Health Care provides quality and compassionate clinical services to the patient in their home, assisted living facility or other residential facility.   

Our qualified psychiatric nurses will work directly with the patient’s physician to ensure the proper treatment is being received by the patient. Depending on the patient’s needs, the treatment team may also include a social worker, physical therapist, occupational therapist, or speech therapist.     

Our treatment team will:
  • Assess mental health needs.
  • Develop & update the patient’s specific treatment plan.
  • Teach & support the patient’s caregivers.
  • Help the patient to be as independent as possible.
  • Assist patients with self-care activities.
  • Have contact with the family, caregiver and physician.
  • Work to avoid rehospitalization.

What is involved in a home visit? A home visit varies based on the patient’s needs. Most visits last 30–60 minutes and the nurse may provide some of the following services:

  • Assess for compliance, side effects, and response to medications.
  • Check vital signs and general physical health.
  • Safety assessment of the home.
  • Teach patient, caregiver and family about the illness and medications.
  • Help patient adjust to being at home.
  • Crisis intervention and counseling.
